Melvin, is a successful romance writer, as well as, a cranky, bigoted, agoraphobic man with OCD who dislikes everyone he meets. His life is turned upside down when his neighbor, an artist named Simon, who Melvin has mercilessly taunted for being gay, is hospitalized after being attacked and Melvin must care for his dog, Urdell. Carol, the only waitress who will serve Melvin at the one eaterie he'll dare, leaves work to care for her severely asthmatic sick son. This forces Melvin to consider medication to change his ways and encourage her return. Carol, too, must change her opinion of Melvin, when she finds out that he can help her son. It's when Marvin, Simon, and Carol take a road trip to Maryland that they find out about each other.
